Licensed Practical Nurse (LPN)

Overview The primary purpose of this LPN position is to provide direct nursing care to residents and supervise the day‑to‑day nursing activities performed by nursing assistants. Supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations governing the facility, and as may be required by the Director of Nursing Services or Nurse Supervisor. Responsibilities Administrative Functions Direct the day‑to‑day functions of nursing assistants in compliance with facility regulations. Ensure nursing personnel comply with written policies and procedures. Review policies, procedure manuals, and job descriptions and recommend revisions. Plan shift services, programs, and activities with staff. Maintain current Nursing Service Procedures Manual. Provide written and oral reports on shift activities. Coordinate with other resident services for total care. Participate in quality assurance and facility surveys. Review and update resident discharge plans and discharge procedures. Assist in planning nursing components of discharge plans. Interpret policies for personnel, residents, visitors, and agencies. Admit, transfer, and discharge residents. Complete accident/incident reports and maintain charge slips. Charting and Documentation File required record‑keeping forms upon admission, transfer, and/or discharge. Encourage physicians to review, order, and sign treatment plans. Record telephone orders and transcribe to resident charts. Document nursing notes reflect care provided and resident response. Complete accident/incident reports and submit to Director. Record diet orders and report discrepancies to Nurse Supervisor. Complete transfer forms and documentation for unauthorized discharges. Sign and date all entries in resident medical record. Drug Administration Functions Prepare and administer medications per physician orders. Verify resident identity before medication administration. Prevent medication mix‑ups between residents. Maintain adequate floor stock of medications and supplies. Order medications, supplies, and equipment as necessary. Ensure accurate narcotic records. Report drug discrepancies to Nurse Supervisor. Review medication cards for completeness and accuracy. Notify physician of automatic stop orders before last dose. Dispose of drugs and narcotics per policy. Personnel Functions Participate in performance evaluations and staffing decisions. Notify Nurse Supervisor of staffing issues. Report absenteeism and evaluate workforce adequacy. Develop assignments and provide leadership to nursing personnel. Conduct daily rounds and report problems to Nurse Supervisor. Supervise nursing aide trainees under a licensed nurse. Meet regularly with shift personnel to address issues. Ensure compliance with department policies and procedures. Facilitate inter‑departmental collaboration for nursing services. Create a positive, calm environment on unit. Handle complaints and grievances fairly and report to Nurse Supervisor. Enforce disciplinary actions without discrimination. Oversee shift hand‑over and report occupational exposures. Nursing Care Functions Inform staff of new admissions and room assignments. Ensure rooms are ready for new admissions. Orient new residents and families to the facility. Collaborate with physicians and coordinate diagnostics. Consult on resident care with physician. Review charts for treatments, medications, and diets. Implement and maintain nursing objectives and plan of care. Verify CNAs administer treatments correctly. Cooperate with social and activity programs. Notify physician and relatives of changes in condition. Carry out rehabilitative programs. Maintain cleanliness of service areas. Administer professional services within scope of practice. Use restraints per policy and patient plan. Collect specimens, record vital signs, and monitor ill residents. Check visitor food for dietary appropriateness. Ensure staff adheres to care plans. Check residents unable to call for help regularly. Assist with transportation and discharge logistics. Inform families of resident death and arrange funeral services. Staff Development Lead training, orientation, and in‑service classes. Standardize work methods and train staff on exposure risk. Attend external and internal training programs. Safety and Sanitation Ensure personnel follow safety regulations and dress codes. Assist in identifying exposure tasks. Maintain supply of personal protective equipment. Keep work areas clean and safe. Implement infection control and isolation precautions. Conduct fire safety and disaster drills. Equipment and Supplies Recommend equipment and supply needs. Maintain adequate stock levels. Implement safe usage procedures. Care Plan Functions Review and ensure adherence to care plans. Report observations that may require care plan changes. Ensure CNAs are aware of care plans. Resident Rights Maintain confidentiality of resident information. Ensure residents are treated fairly, kindly, and with dignity. Provide care in privacy and knock before entry. Educate staff on resident rights, including refusal of treatment. Record and resolve complaints and grievances. Working Conditions Work in nursing service area and office spaces. Experience frequent interruptions and emotional demands. Communicate across departments and with visitors. Work beyond normal hours when needed. Handle emergencies and hazardous chemicals. Education Graduate of an approved LPN/LVN program. Participation in continuing education is desirable. Experience On‑the‑job training provided. Specific Requirements Current, active LPN/LVN license in state. English proficiency (read, write, speak). Ability to make independent decisions. Ability to interact tactfully with diverse stakeholders. Knowledge of nursing practices, laws, and regulations. Ability to plan, organize, and interpret programs and policies. Patience and enthusiasm for handling difficult residents. Willingness to examine new methods and incorporate them. Physical and Sensory Requirements Intermittent movement throughout workday. Speak and write in clear English. Handle mental and emotional stress. Use sight/hearing or prosthetics as needed. Work independently and adapt to residents’ needs. Meet general health requirements. Operate office and medical equipment. Lift up to twenty‑five pounds.
