This role plays a vital part in strengthening clinical partnerships, supporting our referral network, and ensuring seamless transitions for patients into inpatient rehabilitation care.What we're looking for:Clinically licensed professional (RN, LVN, PT, OT, ST, LMSW, or RT)Previous IRF (Inpatient Rehabilitation Facility) experience requiredStrong understanding of inpatient rehabilitationExperience completing Pre-Admission Screening (PAS)Relationship-driven, adaptable, and clinically confidentComfortable working in a dynamic, evolving marketAs a Clinical Liaison, you will:Serve as a key clinical resource within our community, fostering relationships with referral partners, case managers, physicians, and healthcare professionals.Complete thorough pre-admission assessments, review medical records, interview patients and families, and determine clinical appropriateness for admission into our IRF (Inpatient Rehabilitation Facility).Ensure compliance with CMS/IRF regulations and consistently communicate admission status with internal teams and external partners.Provide education and support to referral sources, patients, and their families regarding services, care expectations, and rehabilitation pathways.Represent Acute Rehabilitation Hospital of Plano at community events, educational opportunities, and professional networks.Collaborate closely with Business Development, Admissions, and Clinical teams to support operational excellence and high-quality patient care.Why Join Our TeamAt Acute Rehabilitation Hospital of Plano, we value our team members and invest in their growth and wellbeing.
