The HUD / Intake Coordinator is responsible for coordinating intake, assessment, and referral services for individuals and families seeking housing assistance and supportive services. This position serves as the first point of contact for clients and plays a vital role in connecting participants with resources that support housing stability, mental health care, substance use disorder treatment, and overall well-being.

The Intake Coordinator ensures compliance with HUD regulations, maintains accurate client records, conducts eligibility screenings, and collaborates with community partners and service providers to ensure participants receive appropriate services. The position requires strong organizational skills, compassion, confidentiality, and the ability to work effectively with diverse populations experiencing housing instability, mental health challenges, substance use disorders, domestic violence, and other barriers to self-sufficiency.

Detailed Duty Statement:

Client Intake and Assessment

• Serve as the primary point of contact for individuals seeking housing and supportive
  services.
• Conduct intake interviews and comprehensive assessments to determine eligibility for
  HUD-funded programs and other available resources.
• Gather, verify, and maintain participant documentation required for program enrollment.
• Explain program requirements, participant rights, responsibilities, and available services.
• Complete all intake forms, releases of information, and required documentation
  accurately and timely.

Housing and Service Coordination

• Coordinate referrals to housing programs, shelters, transitional housing, permanent
  supportive housing, and other community resources.
• Assist participants in accessing supportive services, including mental health treatment,substance use disorder treatment, medical care, employment services, education, andbenefits assistance.
• Collaborate with case managers, clinicians, housing specialists, and community partnersto ensure coordinated care and service delivery.
• Monitor participant progress and maintain regular communication with service providersand stakeholders.

HUD Compliance and Documentation

• Maintain participant files in accordance with HUD regulations, agency policies, and
  confidentiality requirements.
• Enter and update client information in HMIS (Homeless Management Information
  System) and other required databases.
• Ensure accurate record keeping and timely submission of reports, assessments, and
  program documentation.
• Assist with audits, monitoring visits, and compliance reviews as needed.
• Track participant enrollment, service utilization, and program outcomes.
  Participant Support and Advocacy.
• Provide information, support, and guidance to participants throughout the intake and
  enrollment process.
• Advocate for participants in accessing housing, behavioral health services, and
  community resources.
• Support individuals experiencing mental health concerns, substance use disorders,
  domestic violence, and other barriers to housing stability.
• Maintain professional boundaries while fostering a welcoming and supportive
  environment.

Administrative Duties

• Answer phones, respond to inquiries, and schedule appointments.
• Coordinate intake schedules and maintain calendars for assessments and orientations.
• Prepare reports, correspondence, and program-related documentation.
• Participate in staff meetings, training, and professional development activities.
• Perform other duties as assigned to support program operations and participant
  success.

Qualifications Needed:

Education

• Associate's degree in Social Work, Human Services, Psychology, Counseling, Public
  Administration, or a related field preferred.
• Relevant work experience may be substituted for formal education.
  

Experience

• Minimum of one (1) year of experience in intake coordination, case management,
  housing services, social services, behavioral health, substance use treatment, or a
  related field preferred.
• Experience working with HUD-funded programs, homeless services, domestic violence programs, or community-based organizations is highly desirable.
• Experience working with vulnerable populations, including individuals experiencing

homelessness, mental health challenges, and substance use disorders.

Knowledge, Skills, and Abilities

• Knowledge of HUD regulations, housing programs, and community resources.
• Understanding of mental health disorders, substance use disorders, trauma-informed
  care, and recovery-oriented services.
• Strong interviewing, assessment, and customer service skills.
• Excellent verbal and written communication skills.
• Ability to maintain confidentiality and handle sensitive information.
• Proficiency in Microsoft Office applications and data management systems, including
  HMIS.
• Strong organizational skills with the ability to manage multiple priorities and deadlines.
• Ability to work independently and collaboratively as part of a multidisciplinary team.
• Demonstrated cultural competency and ability to work effectively with diverse
  populations.

Certifications and Requirements

• Valid California Driver's License and proof of automobile insurance, if driving is required.
• Successful completion of background clearance and any agency-required screenings.
• CPR/First Aid certification preferred.
• Training in trauma-informed care, motivational interviewing, mental health first aid, or
  substance use disorder treatment is preferred.
