MDS Coordinator - RN (Registered Nurse)

Nazareth Home is seeking an experienced and detail-oriented MDS Coordinator - RN (Registered Nurse) to RAI/MDS program in a skilled nursing environment. This position is at our Clifton campus; 2120 Payne St. Join a team dedicated to quality care and compliance excellence!

What We Need You To Do:

• Assist in implementing the RAI/MDS process with federal regulations.
• Participate in care planning and work closely with the nursing department for quality care.
• Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA's and Care Plan) in accordance with current Federal and State Regulations.

Key Responsibilities

• Ensure timely and accurate completion of assessments (OBRA, PPS, Managed Care)
• Coordinate with nursing, therapy, and billing teams to support resident care and reimbursement accuracy
• Participate in the utilization review, case mix management, and Triple Check processes
• Ensure compliance with federal and state regulations, including MDS submission requirements
• Develop and oversee individualized care plans in collaboration with the interdisciplinary team
• Support audits, surveys, and Medicare/insurance reviews
• Provide leadership and guidance to the MDS team and nursing staff

Required Education and Experience

• Graduate from an accredited school of Nursing
• Licensed by the Kentucky Board of Nursing
• Current CPR certification
• One year minimum of experience as a charge nurse in a Long-Term Care setting, preferred
