Summary of position
The Clinical Intake Specialist-LPN, under the supervision of a Registered Nurse (RN), serves as the primary point of contact for individuals with complex care needs. Key responsibilities include outreach, scheduling, and conducting initial in-person visits. The role involves performing comprehensive needs assessments and completing follow-up visits on a quarterly, annual, or as-needed basis when significant changes occur. The Clinical Intake Specialist-LPN provides options counseling, develops personalized care plans, identifies potential risks and safety concerns, and coordinates with the ongoing Case Manager to ensure continuity of care. As a vital member of the care team, they are required to report any potential abuse or neglect, collaborating with the Case Manager and leadership to address immediate concerns. Additionally, the Clinical Intake Specialist-LPN educates participants, caregivers, and family members on available care options and programs.
Essential Functions
- Conduct initial face-to-face visits with referred individuals to gather relevant health and personal information.
- Performs complex needs assessments to evaluate the medical, behavioral, nutritional and social needs of clients.
- Conduct quarterly, annual, and significant change in condition visits to reassess and update clients' needs and care plans.
- Provide options counseling to clients and their families, offering information and guidance on available services and supports.
- Develop and set up individualized person-centered Plans of Care and Service Plans based on the assessments and counseling sessions.
- Advice and counsel participants on cost-effective service planning.
- Maintain accurate and timely documentation of all assessments, visits, counseling sessions, and person-centered plans of care and services plans in accordance with regulatory and organizational standards.
- Collaborate with other healthcare professionals, community resources, and service providers to ensure comprehensive and coordinated care for clients.
- Ensure compliance with all applicable laws, regulations, and organizational policies and procedures.
- Participate in Interdisciplinary (IDT) meetings as necessary.
- Report on any potential Critical Incidents and safety concerns that may put the individual participant in danger.
- Triage and refer participants to the appropriate level of care based on their needs and per program and regulatory guidelines.
Position Requirements
- Requires an associate degree in nursing from an accredited institution or certificate or diploma from a state-approved practical nursing program.
- Requires current and valid License Practical Nurse (LPN) license.
- Minimum of three (3) years of nursing experience, preferably in an intake, triage unit or assessment role.
- Experience in a similar role or in community health, home health, or case management is preferred.
- Knowledge of Medicaid, Medicare and/or waiver programs preferred.

- Excellent interpersonal and communication skills.
- Ability to develop and implement individualized care plans.
- Strong organizational and time management skills.
- Proficiency in using electronic health records (EHR) and other relevant software.
Location
This is a hybrid role. Applicants must be able to commute to or travel to Ben Hill and Irwin County, GA , when required.
Travel/Work Environment
- Frequent travel to clients' homes or other locations as required.
- Must have and maintain a valid state driver’s license, automobile insurance coverage, and access to an automobile.
- Work is performed in a variety of settings, including client homes, community settings, and office environments.
