Your Mission The Resident Care Coordinator supports daily operations and care delivery for Assisted Living and Memory Support communities by coordinating shift activity, communicating across teams, supporting task execution, and ensuring smooth resident experiences. This role acts as the on‑floor liaison between care staff, clinical leadership, families, and residents. Key Responsibilities Shift Coordination & Communication Serve as the primary contact for shift‑level coordination, including assignment updates, pendant/floor checks, and daily task completion. Maintain consistent communication between shifts, ensuring staff are informed regarding resident needs, events, and community updates. Assist with immediate coverage needs by helping identify resources and notifying leadership of staffing concerns. Resident Support & Non‑Clinical Care Coordination Observe resident well‑being, document changes, and report concerns promptly to the Clinical Nurse Manager. Assist with scheduling and coordination of ancillary medical services (labs, podiatry, physician visits). Communicate with families regarding non‑clinical needs, appointments, and routine updates. Operational Support & Compliance Tasks Maintain medication carts/rooms for organization, cleanliness, and audit readiness. Perform monthly environmental, safety, and infection control checklists and submit findings to leadership. Support onboarding by serving as a peer mentor and guiding new caregivers through daily routines. Dementia‑Capable and Caring Community Practices Deliver care using dementia‑capable and trauma‑informed principles, maintaining awareness of sensory, emotional, and behavioral needs. Apply techniques that promote calm, engagement, and trust for residents with cognitive impairment. Participate in life enrichment and memory‑support activities that foster social connection, purpose, and cognitive stimulation. Contribute to a caring community culture by promoting empathy, patience, and meaningful resident relationships. Teamwork and Professional Conduct Collaborate with fellow caregivers, nurses, and other departments to ensure coordinated, consistent resident care. Demonstrate reliability through regular attendance, punctuality, and adherence to assigned schedules. Participate in training and continuing education, including dementia‑capable care and caring community development. Your Qualifications High School diploma or general equivalency diploma (GED) Arizona Certified Caregiver required. At least three years of experience in Assisted Living or Memory Care Must have a current CPR certification and First Aid certification. Must be able to obtain a Maricopa County food handlers card. Possess current or have the ability to obtain a Level 1 Fingerprint Clearance Card is required.
