Registered Nurse, Home Health - Lancaster County

Nursing Position

Full-time (72hours biweekly) Monday through Friday, three shifts per week from 9am-9pm Every 4th weekend, 2 holidays per year Must have prior home health experience! Sign-on bonus eligible!

Department Summary

WellSpan VNA Home Care services 5 counties throughout Central Pa. Our average daily census is 900 patients. Care is provided to those 18 years and older in their homes. Services provided include nursing, physical therapy, occupational therapy, home health aides, social workers, and nutritionist services. We specialize in infusion therapy, wound and ostomy care, LVAD's, and lymphedema.

General Summary

Provides professional nursing services to individuals in the home through a process of nursing assessment, planning, care, and evaluation in accordance with physicians' orders, the established plan of care, the patient's diagnosis, corporate policy and procedures and the Nurse Practice Act. Carries primary care management functions for assigned patients. Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care. May require specialized skills depending on VNA assignment.

Duties and Responsibilities Essential Functions:

• Independently recognizes subtle changes in condition and modifies plan of care accordingly. Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care. Initiate's emergency action in response to acute risk situations.
• Independently organizes and prioritizes time based on higher acuity patient assignments and increased workload.
• Evaluates patient care and promotes goal-directed change to achieve patient outcomes. Able to discuss, document and apply Performance Initiatives and outcomes at the VNA level as they relate to the WellSpan Bold Goals.
• Independently evaluates and monitors the effectiveness of interventions and progress toward outcomes for more complex groups of patients. Initiates interdisciplinary collaboration in evaluation of patient progress. Identifies and facilitates individualized patient needs for transfer/ discharge and includes patient/family in decisions.
• Begins reading current literature. Express interest in evidence-based research and its application to best practice. Understands clinical relevance and its impact on patient outcomes.
• Seeks learning opportunities and resources beyond own work environment and incorporates into practice the new knowledge and skills acquired. Required education and competencies completed, including state mandated continuing education credits for licensure renewal. Encouraged to share pertinent information and job knowledge, related to readings, educational programs, and participation in professional organization activities.
• Actively promotes an environment conducive to professional practice, interdisciplinary collaboration, and patient and staff safety.
• Demonstrates leadership at point of care.

• Variation of duties may result based on the assigned customer care unit(s). This may include differences in the specific clinical skills required of the RN. SYS-NURS-L Nursing Education Requirements Policy outlines current certification(s) required for specific areas of nursing practice and is considered to be part of the job description.
• In addition to requirements specified in the license and certification section below, current BLS certification for all areas of nursing practice is required.Current certification may be required for the following areas of nursing practice:Emergency Care Center ACLS, PALSIntensive Care Center & MSCC Lead ACLS, PALSL&D ACLS, NRP, - Experienced L&D nurses will obtain within 6 months and new L&D nurses will obtain at 1 year of experienceNICU NRPPEDS PALSPCC/PACU/ENDO ACLS, PALSProcedural ACLS

Common Expectations:

• Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
• Demonstrates a commitment to patient, visitor, and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
• Enhances professional growth and development through advanced/continuing education to maintain up-to-date knowledge, skills, and competencies.
• Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
• Provides outstanding service to all customers using caring behaviors; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

Qualifications

Minimum Education:

• Diploma Program Required or
• Associates Degree in Nursing, RN Required
• Bachelors Degree Nursing (BSN) Preferred

Work Experience:

• 1 year Previous experience with the same patient population. Required or
• Current VNA LPN transitioning to RN. Required

Licenses:

• Licensed Registered Nurse Upon Hire Required or
• Registered Nurse Multi State License Upon Hire Required and
• Basic Life Support Upon Hire Required

Driver's License Statement:

Must possess a valid driver's license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.
