Patient Care Associate

Job Summary: Patient Care Associate (PCA) identifies patients eligible for remote monitoring, enrolls them in the program, monitors their data, and coordinates their care with their physicians. Responsibilities Patient Onboarding – Identify patients eligible for remote monitoring. Enroll patients at their physicians’ practices, at patients’ homes, or remotely. Assist with device setup, and train patients on the purpose, benefits, and usage of RPM. Device Management – Ensure devices are properly configured, calibrated, and maintained. Coordinate device replacements or upgrades as needed. Email, phone, or live chat with the technical team for issues, troubleshooting, and problem resolution regarding patient devices. Data Management – Oversee the collection, transmission, and storage of patient data. Monitoring and Analysis – Monitor patient data, analyze trends, identify abnormalities or significant changes in patient health status, and elevate issues to appropriate healthcare providers for intervention. Patient Engagement – Provide ongoing support, encouragement, and communication with patients and/or caretakers to promote adherence to RPM protocols and healthy behaviors. Documentation – Provide routine, timely, and complete documentation into the RPM platform and physicians’ electronic health records. Collaboration – Liaise and collaborate with physicians and their staff to coordinate patient care and optimize the use of RPM within the broader healthcare ecosystem. Requirements & Skills Professional skills : High level of motivation, excellent verbal and written communication skills, strong customer service skills, ability to meet deadlines, maintain confidentiality of information, and adaptability to variable work environments. Technical skills : Proficient in Microsoft Office and the ability to work with emerging technology. Requirements : Must have a reliable mobile phone, laptop, internet connection, and transportation. In-person visits to physician practices and patient homes are required. Education : Associate or bachelor’s degree, or equivalent training and experience. Preferred experience : Prior work in healthcare facilities, familiarity with electronic health records, and medical terminology. #J-18808-Ljbffr
