Job Description

Job Title: Care Manager – IP (Inpatient) RN
Location: Bronx, NY 10451
Duration: 13 weeks
Schedule: Full-Time, 35 hrs/week (Monday–Friday, 9:00 AM–5:00 PM)

Pay Rate: From $2,900/week for travel & From $2,350/Week for local.

Required Qualifications:

Active New York RN License
Bachelor's Degree in Nursing (BSN)
Minimum 5 years of clinical RN experience
Experience in Care Coordination, Utilization Review, and Discharge Planning
Computer literacy
BLS Certification
PRI & Screen Certification (required at submission)
NYSED Primary Source Verification

Preferred Qualifications:

Master's Degree in Nursing
Case Management Certification
Leadership experience

Key Responsibilities:

Inpatient Care Management
Care Coordination
Utilization Review
Discharge Planning
Collaboration with multidisciplinary teams to ensure quality patient care
