  • Conduct thorough evaluations of patients' psychosocial needs, strengths, and coping strategies through detailed interviews and observational methodologies.

  • Provide evidence-based therapy to address a range of mental health issues, including anxiety, depression, grief and loss, and adjustment disorders, particularly in the context of terminal illnesses.

  • Implement crisis intervention strategies in high-stakes scenarios, ensuring patient safety and emotional support during critical moments.

  • Develop and facilitate complex discharge planning to ensure seamless transitions from hospice care to home or other supportive environments.

  • Consult with a variety of healthcare providers to ensure an integrated approach to care, harmonizing psychological resources with physical health interventions.

  • Document all patient interactions accurately in electronic medical records according to HIPAA and organizational policies, promoting transparency and continuity of care.

Patient Population / Client Focus\n

Your primary focus will be on patients in hospice care, which presents unique challenges and requires cultural sensitivity. You will work with individuals facing chronic and terminal illnesses, alongside their families and loved ones, addressing their emotional, psychological, and practical support needs during one of life’s most challenging times. Building a strong rapport and understanding each family’s dynamics is essential for effective intervention.

Qualifications\n

  • Master’s degree in Social Work from an accredited institution.

  • Current and active California state license as a Licensed Clinical Social Worker (LCSW).

  • A minimum of 2 years of relevant clinical experience, particularly in a healthcare or hospice setting.

  • Demonstrated capacity to maintain professional boundaries and uphold confidentiality in all patient interactions.

  • Exceptional verbal and written communication skills, enabling effective interactions with patients, families, and interdisciplinary team members.

Compensation and Benefits\n

We offer a competitive annual salary ranging from USD 120,000 to 130,000, commensurate with your experience and qualifications.
