CASE MANAGER (RN/LIC)

Responsibilities The Case Manager will assess, plan, implement, coordinate, monitor, and evaluate the patient’s plan of care from admission to post‑discharge. They will use clinical knowledge and critical thinking skills to coordinate and implement a discharge plan that meets the patient’s needs and ensures a seamless, effective, and efficient transition of care across the continuum. They will coordinate, negotiate, manage, and implement the care of complex patients to facilitate quality and cost‑effective patient outcomes, working collaboratively with interdisciplinary staff internally and externally, and participating in quality improvement and evaluation processes related to care management.
