Founding Care Navigator – Patient Care & Coordination

Location: New York, NY – Onsite

Schedule: Monday–Friday, 9:00 AM–5:00 PM

Compensation: $90,000–$135,000 base salary + equity

About the Role

We are looking for a Founding Care Navigator to become a trusted advocate and primary point of contact for a growing panel of senior patients and their families.

This is a unique opportunity for an experienced RN who enjoys hands-on patient care, solving complex healthcare challenges, and building something from the ground up.

You will initially focus on direct patient care and care coordination , while also helping us develop the workflows, processes, and standards that will become the foundation of our future care team.

Over time, this role has the potential to grow into a clinical leadership position , including helping recruit, train, mentor, and oversee a growing team of Care Navigators.

What You'll Do

Patient Care & Care Coordination

• Own the care experience for a panel of senior patients and their families.
• Develop individualized care plans based on each patient's health, goals, and circumstances.
• Coordinate care across physicians, specialists, hospitals, pharmacies, insurers, and other healthcare partners.
• Manage post-hospitalization follow-ups and ongoing care needs.
• Coordinate referrals, appointments, and necessary services.
• Support medication reconciliation and identify potential gaps or issues.
• Help patients and families navigate insurance issues, including denials, prior authorizations, and appeals.
• Maintain consistent communication with patients and family members.
• Attend or support appointments when appropriate.
• Follow through on important action items and ensure care plans are completed.
• Serve as a consistent, trusted resource for patients and their families.

Help Build the Care Model

• Independently manage an initial patient population while maintaining a high standard of care.
• Identify recurring challenges and opportunities within the patient experience.
• Help create scalable workflows, processes, scripts, escalation protocols, and handoffs.
• Document best practices and lessons learned from working directly with patients.
• Partner with technology and product teams to improve care delivery.
• Provide clinical input into tools, workflows, and technology.
• Help establish the foundation for a scalable care-delivery model.

Help Build the Future Care Team

As the organization grows, you will have the opportunity to:

• Help define the skills and backgrounds needed for future Care Navigators.
• Participate in recruiting and onboarding new team members.
• Train and mentor Care Navigators.
• Help establish quality standards for patient care.
• Contribute to the development of team processes and best practices.
• Eventually help build, lead, and oversee a growing care team .
• Serve as a clinical leader and advocate for patients as the organization scales.

Your First 90 Days

Your first 90 days will primarily focus on patient care, building trust, and learning where we can improve the healthcare experience.

You will:

• Build strong relationships with your initial patients and families.
• Independently manage your patient population.
• Understand each patient's needs, goals, and challenges.
• Develop clear and actionable care plans.
• Ensure referrals, appointments, medications, follow-ups, and insurance issues are addressed.
• Establish yourself as a trusted resource for patients and families.
• Identify friction points within the healthcare system.
• Find opportunities to improve the patient experience.
• Begin documenting processes and best practices that can eventually be scaled across the broader care team.

Success in your first 90 days means your patients feel genuinely cared for, families know they have someone in their corner, and every patient has a clear plan for what comes next.

Required Qualifications

• Active RN
• Demonstrated ability to work independently and take full ownership of a patient population with minimal day-to-day direction.
• Strong clinical judgment and ability to assess complex situations, prioritize needs, and develop individualized care plans.
• Comfortable working in a startup environment where processes and workflows are still being built.
• Proactive, entrepreneurial mindset with a willingness to take ownership and solve problems.
• Growth mindset and willingness to learn, seek feedback, experiment, and continuously improve.
• Passion for improving the healthcare experience for older adults and their families.
• Comfortable balancing direct patient care with building scalable processes and workflows.
• Strong interest in healthcare technology and willingness to use AI and other tools to improve efficiency and patient outcomes.
• Demonstrated potential to grow into a leadership role, including hiring, mentoring, developing, and eventually overseeing a team.
• Comfortable collaborating with clinical, operational, product, and technology teams.
• Willingness to work onsite five days per week.
