Job Description

As a Care Coordinator at Right at Home, you will play a vital role in coordinating care for our clients by maintaining strong relationships with caregivers and clients and ensuring continuity of care. This is a fast-paced position that requires excellent organizational skills, flexibility, and the ability to problem-solve quickly.

This role also includes participation in our on-call rotation , which provides additional compensation. During assigned on-call shifts, you will be responsible for responding to urgent staffing needs during evenings, weekends, and holidays to ensure uninterrupted client care.

Additionally, this position represents Right at Home in the community. Candidates must be willing to attend occasional weekend networking, educational, and community events as needed.

Key Responsibilities

• Each Care Coordinator will be assigned a caseload of clients and will be responsible for coordinating their ongoing care. This includes ensuring supervisory visits and plan of care updates are completed on time, maintaining communication with clients and caregivers, and helping ensure each client receives consistent, high-quality service.

• Manage and adjust schedules to accommodate changes in client needs, caregiver availability, call-offs, and emergency staffing situations.

• Communicate professionally with clients, families, caregivers, referral partners, and office staff regarding scheduling and care needs.

• Participate in the office on-call rotation, responding promptly to after-hours staffing emergencies. Additional compensation is provided for assigned on-call shifts.

• Assist with finding coverage for open shifts, including evenings, weekends, holidays, and last-minute staffing needs.

• Support recruitment, onboarding, and caregiver retention efforts as needed.

• Represent Right at Home at community outreach events, health fairs, networking opportunities, and educational events, including occasional evenings and weekends.

• Ensure compliance with company policies, procedures, and all applicable regulatory standards.

• Perform other administrative and coordination duties as assigned.

Qualifications

• Previous experience in care coordination, healthcare, home care, staffing, or a similar role is preferred.

• Strong organizational and time management skills with exceptional attention to detail.

• Ability to multitask and remain calm under pressure in a fast-paced environment.

• Excellent verbal and written communication skills.

• Strong customer service and problem-solving abilities.

• Ability to work independently while collaborating effectively with a team.

• Must be willing to participate in an on-call rotation, including evenings, weekends, and holidays.

• Must be willing to attend occasional weekend and evening community events and marketing activities as needed.

• Experience with WellSky or similar home care software is preferred.

• A compassionate, dependable, and professional demeanor.

• Valid driver's license and reliable transportation.
