It is a non‑profit organization located in historic Bolivar County, Mississippi, and the first Federally Qualified Community Health Center (FQHC) in the United States. DHC provides high‑quality, affordable preventive and primary health care to underserved communities across the Mississippi Delta. Position Summary We are seeking a full‑time, mission‑driven Family Physician to join our clinical team at Delta Health Center. The on‑site role is based in Mound Bayou, Mississippi and involves providing comprehensive primary care services from pediatrics to geriatrics. The Physician will diagnose and treat a broad range of acute and chronic conditions, address urgent care needs, and actively support community health initiatives. Collaboration with interdisciplinary care teams to ensure patient‑centered, high‑quality outcomes is central to this role. Duties and Responsibilities Provide comprehensive primary care to patients of all ages, with a focus on preventive health, chronic disease management, and health promotion. Evaluate, diagnose, and manage acute and chronic illnesses—including hypertension, diabetes, asthma, cardiovascular disease, and infectious conditions—using evidence‑based clinical practices. Address urgent and walk‑in medical needs within the FQHC setting, providing timely assessment and treatment. Perform comprehensive patient histories and physical examinations, conduct risk assessments, order and interpret diagnostic and therapeutic tests (e.g., labs, imaging, EKGs), and manage follow‑up care. Prescribe medications, therapies, and referrals in accordance with clinic policy, formulary guidelines, and applicable Mississippi State regulations. Coordinate chronic disease management, developing individualized care plans and engaging care coordinators and specialists as needed. Deliver preventive health services, including immunizations, screenings, and counseling, and educate patients and families on lifestyle modifications. Participate in and support DHC’s community health programs, outreach events, and population health efforts. Collaborate within a multidisciplinary team—including nurses, mid‑level providers, behavioral health staff, and social workers—to ensure whole‑person, seamless patient care. Recognize conditions requiring specialist intervention and facilitate timely referrals and consultations. Maintain accurate, thorough, and timely electronic health records in compliance with FQHC, HIPAA, and clinical documentation standards. Requirements Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school. Completion of an ACGME‑accredited Family Medicine residency program. Current, valid medical license to practice as a Family Physician in Mississippi, or eligibility to obtain prior to start date. Board certification or board eligibility in Family Medicine (ABFM or AOBFP). Current DEA registration or eligibility to obtain. Minimum of one (1) year of post‑residency clinical experience in a primary care or outpatient setting (new residency graduates with strong clinical training encouraged). Prior experience working in a Federally Qualified Health Center, rural health setting, or with underserved populations is a plus. Technical Skills Experience with Electronic Health Record (EHR) systems; NextGen or similar platforms preferred. Basic proficiency in Microsoft Office applications. Familiarity with telehealth/virtual care platforms (preferred). Core Competencies Clinical Judgment: Strong diagnostic reasoning and the ability to manage complex, multi‑system patients.
