Job Summary:
Hospice Registered Nurse (RN) Basking Ridge, New Jersey. This position involves providing comprehensive hospice care to patients in their homes, including patient assessments, care plan development, interdisciplinary communication, medication management, and ensuring compliance with hospice regulations. The RN will collaborate with the interdisciplinary team to deliver high-quality palliative care and support to patients and their families.

Job Details:

• Location: Basking Ridge, New Jersey
• Schedule: Days, 7.5 hours per shift, minimum of 37.5 hours per week
• Required Qualifications: Associate's (ASN) degree OR Diploma with RN License in New Jersey; current New Jersey RN License; experience using an electronic medical record (EMR); excellent critical thinking and communication skills; valid New Jersey driver's license; BLS certification; at least 2 years of RN experience in a home hospice environment.
• Preferred Qualifications: More than 3 years of RN experience.
• Additional Information: Performs patient assessments, develops care plans, ensures appropriate utilization of services, completes discharge assessments, performs patient education, manages case activities, and participates in performance improvement programs. The candidate must attend bi-weekly interdisciplinary team meetings and participate in agency growth activities.
