Hospice Inpatient Admissions Nurse

Liberty Health seeks a compassionate Hospice Inpatient Admissions Nurse to coordinate and deliver high-quality end-of-life care. In this role, you’ll complete timely admissions, perform comprehensive assessments, and develop individualized hospice care plans in collaboration with physicians and the interdisciplinary team. You’ll educate patients and families, manage complex symptoms, and ensure comfort and dignity throughout the inpatient stay.
