General Summary:The Registered Nurse (RN) provides professional nursing care activities under the direction of a physician/providerin Family Medicine within his/her scope of practice (RCW 18.360.050). The RN practices in a Certified Rural HealthClinic (RHC) environment and supports the delivery of comprehensive, patient-centered primary care services topatients across the lifespan. The RN may not accept delegation of acts not within his/her scope of practice.The RN assists the physician/provider with the provision of care, including but not limited to: recording vital signs,taking height and weight measurements, discussing primary health concerns, performing tests, administeringinjections and medications, assisting with procedures and documentation, entering all patient information, andclinical data in the electronic medical record (EMR), communicating key patient information to thephysician/provider, and anticipating and acting on the patient’s needs proactively.The RN performs nursing assessments, establishes nursing diagnoses or problem lists, develops and implementspatient care plans, provides patient education, and delivers clinically competent, quality patient care. The RN isresponsible for assessing and responding to patient telephone calls and electronic patient messages, includingclinical triage, symptom assessment, prioritization of care needs, and coordination with providers to ensure timelyand appropriate patient care and follow-up. The RN provides clinical guidance to patients within establishedprotocols and the scope of practice and facilitates access to appropriate levels of care.In addition, the RN supports the clinical support team as needed, including collaboration with medical assistants,patient access staff, and other interdisciplinary team members, to maintain efficient clinic workflows and continuity ofcare.Minimum Education, Training & Experience (includes licenses or certifications):Safety Sensitive per WASB5123: YesEducation / Licensure:High school diploma or GED required.Graduate of an accredited school of nursing with an active/current RN license in Washington State.Current BLS certification required.Basic computer knowledge required.Experience:At least one (1) year of recent full-time experience providing direct patient care in an outpatient or acute care medical setting is preferred.Skills/Knowledge/Abilities:Demonstrated knowledge of general nursing care skills (within scope of practice), medications (including reactions and dosage requirements), patients’ rights, universal precautions, infection prevention, body mechanics, medical and nursing terminology, as well as nursing theory, principles, and practices to care for the patient holistically, triage patients in person or over the phone.Requires critical thinking skills, the ability to make good decisions, listen and accept advice from others, demonstrate maturity and good judgment, and the ability to work with regular supervision.Perform various sensory assessments, including vision, speech, smell, touch, manual dexterity, fine motor skills, and hearing, to assess and treat the patient accurately.Promote/deliver excellent customer service to both internal (patients and staff) and external customers (visitors and vendors).Demonstrate positive and effective interpersonal and communication skills with colleagues, patients, families, physicians/providers, volunteers, and the community.Work productively and cohesively within a team and resolve conflicts/differences constructively.Work in a stressful/demanding environment and take appropriate action to ensure the quality of patient care, and demonstrate the ability to cope with both the physical and emotional demands of the job.Maintain a calm and composed manner in emergency/stressful situations, taking appropriate action as necessary.Complies with mandatory, regulatory, and credentialing requirements.Completes initial and annual skills competencies.Demonstrate attention to detail, follow oral and written instructions precisely, and maintain a high degree of accuracy; failure to do so could result in significant harm or loss.Excellent organizational and multi-tasking skills with the ability to anticipate tasks and prioritize assignments.Ability to read, write, speak, and understand English.Experience with using everyday electronic computer tools (e.g., email, data retrieval, voicemail, and internet/intranet) as well as ability to effectively utilize and enter information and clinical data in the electronic medical record (EMR), as well as use/handle all medical equipment utilized in the care of the patient. Plus a copier and a fax.Proficiency with basic computer programs, including Microsoft Word and Excel, and have good typing skills.Adaptability is essential for managing a department's needs with unpredictable volume and resource requirements.ESSENTIAL FUNCTIONS / RESPONSIBILITIES (but not limited to):Collects data/information such as: vital signs, height and weight measurements, and accurately records the patient’s chief complaint, symptoms, history, allergies, and immunizations in the medical chart or electronic medical record (EMR), updates medication reconciliation and allergy information.Enters all required patient information and clinical data into the electronic medical record (EMR) to maintain the clinic's paperless charting.Observes and records the patient’s condition and any changes from the last visit; documents in a timely, legible, and concise manner; and establishes and updates the problem list.Establishes goals/objectives related to the patient's plan of care based on the patient's unique needs; establishes care plan priorities based on the patient's response to treatment and interventions; communicates with the physician/providers regarding the patient’s response to treatment and care.Collects specimens as instructed by the physician/provider.Prepares patients for examination and assists the physician/provider as needed during the examination (e.g., holds the child for injection, turns the patient, hands instruments to the physician, etc.).Obtains, prepares, and sets up supplies and instruments to be used in the care and treatment of patients.Administers injections/vaccines, medications; seeks clarification of unclear verbal or written orders, reports all drug errors or near misses as appropriate, consults with physician/provider when seeking additional information.Performs waived testing as allowable by CLIA (e.g., urine dip, blood sugar, rapid strep).Tracks and routes diagnostic test results to provider(s) for review, adjusts care plan as needed, and communicates test results to patient when deemed appropriate by physician/provider.Reports abnormal values, significant deviations, or key information to the physician/provider promptly.Uses the principles of growth and development to assess each patient’s age-specific needs, provides age-specific treatment and care, and communicates age-appropriate physical and psycho/social assessments.Provides patient education to facilitate recovery, self-care, health maintenance, health promotion, and wellness; documents patient teaching, tools used, and patient understanding.Maintains a clean, well-stocked, safe environment for patients and staff. Keeps patient care and utility areas neat and clean.Answers phone calls, takes messages, and assists the physician/provider or other staff as needed.Participates in departmental and organizational quality management activities.Cooperates with other personnel to achieve department objectives and maintain good employee relations, interdepartmental objectives, and infection prevention policies.Attends various staff meetings and mandatory in-services as required; completes required education on safety, disaster plans, infection prevention, etc.Performs other miscellaneous duties as assigned. #J-18808-Ljbffr Klickitat County Public Hospital District No 1
