Home Health Occupational Therapist OtPathwell Health, a trusted and community-focused home health care provider, is seeking a skilled and compassionate Home Health Occupational Therapist OT to join our team. As an integral part of our organization, you will play a vital role in assessing, planning, organizing, and participating in rehabilitative programs that help individuals with disabilities or developmental delays regain their independence.Responsibilities:Develop, implement, and evaluate plans of care in partnership with clients, representatives, caregivers, and physiciansProvide direct occupational therapy services in accordance with established plans of careEducate and instruct clients, family members, or representatives on rehabilitation care and activities to promote health, safety, and independent livingAccurately document observations, interventions, and evaluations pertaining to client care management and services providedSelect activities that will help individuals learn work and life-management skills within limits of their mental or physical capabilitiesRecommend changes in patients' work or living environments, consistent with their needs and capabilitiesEngage in research activities and participate in formal educational training activities related to therapyPerform other duties as assignedMinimum Qualifications:Current and valid Connecticut Occupational Therapist license in Virginia (required)Recent work experience in home health (preferred)Driver's License, Car Insurance, Car RegistrationStrong clinical assessment, organizational, and communication skillsAbility to handle multiple assignments simultaneouslyAbility to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentationBilingual (Spanish) preferredExcellent oral and written communication skillsExcellent customer relationship skillsPhysical Demands and Work Environment:The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
