Description

• Assess patients in both inpatient and outpatient environments while creating tailored treatment regimens
• Implement therapeutic interventions targeting mobility enhancement, strength building, balance improvement, and functional abilities
• Partner with nurses, physicians, case managers, and rehabilitation specialists to achieve optimal patient outcomes
• Guide patients and their families through therapeutic objectives, aftercare planning, and guided home exercise programs
• Maintain accurate documentation of evaluations, treatments provided, and patient progress in line with facility protocols and regulations
• Engage in care team conferences and interdisciplinary meetings to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications such as neurological or geriatric may be advantageous, though not mandatory

Skills

• Previous professional experience is strongly preferred
