University of Michigan Health is ranked nationally for the care provided to both adult and pediatric cancer patients, as well as many other specialties. Current hematology/oncology clinical pharmacists provide exceptional pharmaceutical care on the individual and population level. If you are passionate about patient care, instructing, mentoring, quality improvement, and clinical research there is no better place.Responsibilities*This pharmacist will:Provide care to patients seen in the ambulatory solid tumor clinics with a focus on breast cancer and melanoma. As an integral member of the multidisciplinary team, collaborate with providers on crafting and personalization of treatment plans for patients with breast cancer, melanoma, and other solid tumor malignancies.Guide incorporation of new treatments in a standardized evidence-based fashion, in conjunction with other members of the solid tumor pharmacist team, including involvement at tumor board discussions.Provide independent visits with patients to educate on complex treatment regimens and provide supportive care. Practice under a Collaborative Practice Agreement for nausea, vomiting, constipation, and diarrhea, as well as participate in a nausea and vomiting patient-reported outcome text messaging program.Coordinate care to promote outpatient delivery of treatments for solid tumors.Collaborate closely with inpatient solid tumor teams to provide patient centered care. Current license to practice pharmacy in the State of Michigan (including a Controlled Substance license), verifiable in LARA; or eligibility to become licensed within 90 days of employment start date.A certificate of completion from an oncology residency, fellowship, or equivalent combination of education, training, and experience is necessary.
