Physical TherapistEnhance Rehabilitation is currently seeking qualified Physical Therapists candidates to join our dynamic team. As an Enhance Rehabilitation Physical Therapist, you will be a part of growing team providing the highest quality care for patients in SNF setting. In this role, you will evaluate and treat patients following an individualized plan of care.Responsibilities include:Providing excellent care for our patientsProviding excellent customer service to our patients and family membersCollaborate with facility staff to ensure excellent care for your patientsEvaluating new patients, developing plans of care and setting measurable, functional goalsTreating patients, answering their concerns and educating them on their injury, progress and probable outcomesSupervising Physical Therapist AssistantsRe-evaluating patient progress, revising plans of care and resetting goalsComplete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
