Home Health Physical Therapist - Drive Patient Independence

HCR Home Care seeks a qualified Physical Therapist to provide home-based PT services in New York. You will evaluate patients, develop and modify plans of care, and document clinical notes in the medical record following physician orders.

Responsibilities include instructing patients/caregivers on prosthetics, orthotics, and durable medical equipment, attending staff meetings, and participating in care conferences to optimize patient outcomes.
