About This Role The Care Manager (RN) role at Centene involves developing and facilitating complex care management activities for members with physical needs, ensuring high-quality healthcare outcomes through personalized care plans and education. Highlights Location: Remote, MI Schedule: Full Time, Monday - Friday, 8am - 5pm Travel: Up to 75% local travel to support members in Wayne and Macomb counties What You'll Do Evaluate member needs and barriers to accessing appropriate care Develop ongoing care plans and collaborate with providers Identify problems/barriers to care and provide interventions Coordinate between members, families, and care provider teams Provide ongoing follow-up and monitoring of member status Offer resource support for local services based on assessments Facilitate care management and ensure timely access to services Perform telephonic, digital, or home outreach to assess needs Document and maintain member information for compliance Educate members and families on disease processes and care options Requirements Education: Degree from an Accredited School of Nursing or Bachelor's degree in Nursing Experience: 2 – 4 years of related experience Licenses & Certifications: RN - Registered Nurse - State Licensure and/or Compact State Licensure required Benefits Competitive pay Health insurance 401K and stock purchase plans Tuition reimbursement Paid time off plus holidays Flexible work schedules (remote, hybrid, field, or office) About Centene Centene is a leading healthcare provider committed to transforming the health of communities one person at a time.
