Recovery-Focused ACT Peer Specialist

St. Luke's is seeking a Certified Peer Specialist to join its Assertive Community Treatment (ACT) Team. The role focuses on outreach, monitoring, assessment, service planning and family consultation to support recovery and community integration. Responsibilities include in-home and community visits, documentation in the EHR, crisis on-call duties, and cross-training with ACT specialties to expand housing and vocational supports.
