Admissions Coordinator - BH Fulton County Nursing Baxter Health Fulton County Hospital Salem , AR 4123 - BH Fulton County Nursing Full time , Days , 8:00am - 4:30pm Nursing Admissions Coordinator - Salem, AR Location Admissions Coordinator SUMMARY: Under the general direction of the Program Director, you will develop an appropriate discharge plan, make recommendations for alternative levels of care, identifycost-effective care options and facilitateappropriate clinical follow-up. This work is done through patient assessment and engagement, chart review and collaboration with the healthcare team. You will monitor patient progress throughout the hospitalization, modifying discharge plans as needed and continually assess medical necessity. The Coordinator will also provide information to post-hospital vendors as needed to ensure ongoing recovery for the patient. Admissions Coordinator JOB REQUIREMENTS: Education: Graduate of an accredited school of nursing or Masters of Social Work. Licensure: Current Arkansas or Compact State Registered Nurse license in good standing, if applicable. Experience: Minimum of 2 years experience in previous Utilization Review or Case Management preferred. Certifications: ACM or CCM Certification preferred. Other: Strong communication, organizational, and multitasking abilities. Familiarity with Medicare/Medicaid guidelines and hospital regulatory compliance. Preferred Education: Bachelor's Degree Admissions Coordinator Safety Sensitive Designation: This position is deemed safety sensitive in accordance with Arkansas Act 593 governing Medical Marijuana.
