Nurse Practitioner, Advanced Practice Provider

Role Nurse Practitioner, Advanced Practice Provider Responsibilities Deliver equitable, value-based primary care to adult Medicare patients, including routine and acute office visits, annual wellness exams, health assessments, care coordination, patient education, and participation in outreach and promotional activities. Conduct home visits as needed and collaborate closely with a multidisciplinary team focused on primary care, population health, and care continuity. Maintain accurate documentation and partner with specialists and hospitals for comprehensive patient management. Qualifications Master's or DNP degree, national certification in Family, Adult-Gerontology, Adult, or Gerontological Nurse Practice, active state license, DEA license, and US work authorization. Preferred experience in primary care, understanding of HCC, ICD-10 coding, and health risk assessments. Bilingual skills and a team-oriented mindset are a plus. Skills Strong clinical knowledge, care coordination, documentation proficiency, patient communication, and collaborative teamwork. Ability to work independently within a team environment and adapt to population health models.
