State Psychiatric Hospital (SPH) Liaison

Help bridge the gap between hospital care and community support.Southwestern Behavioral Healthcare is looking for a full-time State Psychiatric Hospital Liaison to join our team and help coordinate quality care for adults living with serious and persistent mental illness. In this role, you’ll serve as a key connection between State Psychiatric Hospital (SPH) settings and Community Treatment Facilities, coordinating patient transfers, sharing important information, and helping ensure continuity of care throughout the transition process.
