As an Occupational Therapist, you will perform evaluations and treatments as ordered by physicians, ensuring high-quality care according to company, federal, state, and local standards. Essential Activities and Tasks Care Management and Service Delivery (60%) Initiate contact with physicians for orders and treatment plans as needed. Develop and implement patient OASIS assessments, evaluations, and discharge documentation. Perform physical therapy treatments as prescribed, evaluating and treating patients with appropriate techniques and modalities in the home environment. Recommend modifications to the patient’s Plan of Care based on progress toward goals. Educate and instruct patients, families, and the health team on proper techniques, exercise programs, precautions, and self‑care activities for safety. Participate in interdisciplinary team meetings to exchange information on progress and discharge planning. Communicate with patients, families, advocates, caregivers, and physicians regarding progress and discharge plans regularly. Conduct home visits to implement the Plan of Care to improve function and safety, enabling patients to return home at their highest level of independence. Assist in discharge planning by exchanging information with staff, families, and physicians to promote continuity of care. Maintain clean, sanitary, and safe treatment areas and equipment. Administration and Reporting (20%) Maintain current and accurate patient records using EMR and paper/chart documentation per company and regulatory guidelines. People Management and Development (10%) Coach and influence Certified Occupational Therapy Assistants regarding performance expectations. Foster a positive work environment that attracts, retains, and motivates employees. Interview, select, and orient new employees. Define and promote performance expectations for staff. Evaluate job performance, provide developmental guidance, and monitor accountability per Employee Handbook. Participate in planning, scheduling, and overseeing in‑service training, on‑the‑job training, and orientation programs. Assist in staffing plans for absences or vacancies. Respond promptly to staff requests and work product reviews. Quality, Compliance and Risk Management (10%) Maintain compliance with all relevant standards, regulations, and rules. Respect confidential information and follow company policies and procedures. Perform all other duties as assigned. Qualifications Education Current unencumbered license to practice as an Occupational Therapist in the state of Ohio required. Experience One year of experience as an OT in a hospital, long‑term care facility, or home health agency preferred. New graduates welcomed. Experience in caring for geriatric OT patients required. Knowledge of treatment modalities. Other Requirements Ability to read, write, speak, and understand English. Valid driver’s license with acceptable record; insurable under professional liability and crime coverage per insurer.
