After Hours Hospice Triage Registered Nurse

Job Title: After Hours Hospice Triage Registered Nurse Location: Remote / Nationwide, USA. Employee Type: Full‑time (40 hours/week). Compensation: $70,000.00 - $75,000.00 per year, fixed range with a maximum cap of $75,000.00. Hours: 12:00AM – 8:00AM Monday–Friday, every other weekend; 8:00PM – 8:00AM fully remote “after‑hours” coverage. Job Responsibilities Handle all triage calls during assigned business hours. Report all information to the team at end of shift and synchronize electronic documents promptly. Stay current on system memos, updates, emails, and team communications. Respond to incoming telephone calls from patients and families politely and professionally, respecting individual, cultural, and spiritual differences. Make patient‑care decisions in response to incoming calls using current databases and professional nursing judgment; this may include giving advice, directing contact to a physician, or scheduling a visit. Triaged after‑hours calls aiming for problem resolution, patient satisfaction, safety, symptom management, positive outcomes, and reduced preventable ER/hospital admissions. Initiate communication with attending physicians, team members, and agencies when needed. Coordinate and direct after‑hours care for assigned home‑care and hospice regions, including referral coordination, staffing, and scheduling of after‑hours visits (DME/IV supplies, etc.). Review hospice referrals received after hours; assign staff, update, and communicate changes to all disciplines, regional scheduling, and customer service. Follow up with management staff on concerns regarding patient care. Document accurately and promptly each patient call or encounter in real time and maintain up‑to‑date patient records for care coordination. Coordinate and assign medically necessary visits requested by hospice patients/caregivers to meet patient needs. Qualifications Associate degree or higher from an accredited School of Nursing. RN license in the state of employment. Minimum of 2years of experience as a Registered Nurse, including hospice experience. Demonstrated experience using tablets, mobile phones, and EMR software.
