Blue Sky is looking for an experienced professional to coordinate community-based services, support and coach employees, work with families and support teams, and make sure individualized services are delivered safely, consistently, and according to plan. We're hiring a Community Services Coordinator to oversee services and support the employees who provide them. Within our Minnesota 245D program, this position also serves as a Designated Coordinator, with responsibility for ensuring services are person-centered, well-coordinated, properly documented, and delivered in accordance with individual plans and regulatory requirements. This is a hands-on coordination role for someone who can lead people, organize details, solve problems, communicate effectively, and take ownership when something needs attention. What Does This Job Actually Look Like? You'll be responsible for keeping multiple pieces moving at once — people, services, documentation, communication, and day-to-day program needs. You may spend your day: Supporting and coaching employees who provide direct services Helping employees understand expectations and solve problems Coordinating services based on each individual's needs, goals, and support plan Communicating with individuals, families, guardians, team members, and other professionals Participating in service planning and support team meetings Developing, implementing, and monitoring individualized support strategies Training employees on participant-specific needs and service expectations Reviewing documentation and following up when something is incomplete or needs attention Responding to changes in staffing, services, participant needs, or other operational priorities Identifying concerns and determining what needs to happen next Helping ensure services meet Blue Sky standards and Minnesota 245D requirements Working with other Blue Sky leaders to improve service quality and participant outcomes This position combines people leadership, program coordination, administrative responsibility, and regulatory oversight. Because this position serves as a Minnesota 245D Designated Coordinator, however, candidates must also meet the human-services education, training, and experience requirements listed below. Are comfortable leading, coaching, and holding people accountable Communicate clearly and professionally, including when conversations are difficult Can build relationships with people who have different personalities, backgrounds, abilities, and communication styles Stay organized when several things need your attention at once Notice when something isn't right and follow through until it's resolved Can make decisions and solve problems without needing constant direction Balance empathy with clear expectations and accountability Are comfortable with documentation, deadlines, policies, and regulatory requirements Can move between office work, employee support, meetings, and community-based services Are dependable and responsive when others need information or direction Enjoy improving how things work rather than simply maintaining the status quo What You'll Need Ability to successfully complete a Minnesota DHS background study Valid driver's license and driving record that meets Blue Sky requirements Reliable, insured transportation Strong written and verbal communication skills Comfort using computers, email, Microsoft Office, and electronic documentation systems Ability to organize multiple priorities, meet deadlines, and follow through on responsibilities Ability to lead employees while working collaboratively with individuals, families, guardians, coworkers, and other professionals Minnesota Designated Coordinator Qualifications Because this position serves as a Designated Coordinator within our Minnesota 245D program, applicants must meet at least one of the following education and experience combinations: Bachelor's degree in a human services-related field and at least 1 year of qualifying direct-care experience Associate degree in a human services-related field and at least 2 years of qualifying direct-care experience Diploma in a human services-related field and at least 3 years of qualifying direct-care experience OR At least 50 hours of education and training related to human services and disabilities and at least 4 years of qualifying full-time direct-care experience under appropriate supervision Not sure whether your education or experience qualifies?
