Registered Nurse, Home Care

Bryant Staffing Solutions is seeking a dedicated Registered Nurse (RN) for a full-time home care position serving patients throughout Northern New Jersey. This opportunity allows nurses to deliver personalized, one-on-one care in patients' homes while maintaining a structured daytime schedule. The ideal candidate has strong clinical assessment skills, works confidently with minimal supervision, and is comfortable managing a community-based patient caseload. Medical-surgical (Med/Surg) nursing experience is strongly preferred. Previous home health experience is beneficial but not required.

Work Schedule and Territory

• Schedule: Monday - Friday, 8:30 AM - 5:00 PM
• Hours: 40 hours per week
• Weekend Rotation: Every fourth weekend, both Saturday and Sunday
• Holiday Rotation: Two holidays per year (one winter holiday and one summer holiday)
• On-Call: No on-call required
• Territory: Bergen County, Passaic County, and portions of Morris County, NJ
• Mileage Reimbursement: Reimbursed at the applicable IRS standard mileage rate for eligible travel between the first and last patient visits of the day
• Work Environment: Field-based home care with direct patient visits

Key Responsibilities

• Provide skilled nursing care to patients in their homes, including assessments, treatments, and ongoing monitoring.
• Develop and maintain individualized nursing care plans in collaboration with physicians and interdisciplinary teams.
• Perform medication reconciliation, administer prescribed treatments, and educate patients and caregivers.
• Monitor changes in patient conditions and communicate significant findings to appropriate healthcare professionals.
• Coordinate patient visits and ensure care is delivered according to physician orders and authorized service requirements.
• Complete timely and accurate electronic clinical documentation.
• Educate patients and families about disease management, medications, safety, and recovery.
• Supervise home health aides and conduct required supervisory visits.
• Coordinate patient transitions, referrals, and ongoing services.
• Manage assigned patient visits efficiently while maintaining clinical quality and regulatory compliance.

Required Qualifications

• Associate Degree in Nursing (ADN) or Diploma in Nursing.
• Active New Jersey Registered Nurse (RN) license.
• Current American Heart Association (AHA) Basic Life Support (BLS) certification for healthcare providers.
• Minimum six months of RN experience OR at least one year of LPN experience.
• Basic computer proficiency and ability to maintain electronic clinical records.
• Ability to travel throughout the assigned Northern New Jersey territory.
• Availability for the required weekday, weekend, and holiday schedule.
• Medical-Surgical (Med/Surg) nursing experience

Preferred Qualifications

• Bachelor of Science in Nursing (BSN).
• Previous home health, hospice, or community-based nursing experience.
• IV certification.
• Experience independently assessing patients, managing complex conditions, and coordinating care.
