After Hours RN

Join to apply for the After Hours RN/LPN role at EverHeart Hospice Includes rotating every 4th weekend and holiday rotation. Summary of Position The After-Hours Nurse is responsible for direct patient care after business hours. Education and Experience Graduation from an approved School of Nursing. Current RN or LPN licensure in the State of Ohio. Minimum of two years of experience in Medical/Surgical, Oncology or community health nursing. Hospice experience preferred. Special Skills and Requirements Shall have a valid driver’s license. Detail oriented with excellent communication, documentation, and organizational skills. Ability to work independently without supervision. Travel to other sites as needed. Travel may be in inclement weather. Ability to assist with patient care activities such as ambulation, transfers, and positioning. Responsibilities of Position Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching and timely response to patient and family needs, including: Routine homecare visits End‑of‑life/continuous care Referrals and admissions Care center assistance Chart reviews Other duties as assigned Explain hospice services and benefits regarding Medicare, Medicaid and private insurance, and know when to use palliative care interventions and ABNs. Obtain and enter orders from Medical Directors and Physicians when deemed appropriate. Order appropriate Durable Medical Equipment (DME) for patients. Facilitate continuity of care by coordinating admission of home‑care patients to in‑patient settings (hospital, nursing home), as needed. Complete all documentation per organization policy, accreditation bodies and governing requirements.
