Job Overview The Patient Relations and Retention Liaison position supports the Senior/Medicare population, including the AltaMed Plus Product, to provide a superior patient engagement experience for telephonic targeted outbound campaigns, in-person patient welcome, ongoing point of contact for service/care coordination, opt-in process, and health plan transition. The Patient Relations/Liaison is responsible for calls to identified patients/prospects, uses patient engagement skills to discuss benefits of AltaMed services, schedules Meet and Greets as appropriate, welcomes patients at clinic for initial appointment, assists patient with clinic team introductions, post clinic meeting with questions and/or concerns and explores interest in AltaMed and opt in options for AltaMed Plus in real time, if required. In this role, the Liaison helps the patient navigate through health care needs and supports providers/ clinical team with relevant patient insights for care planning purposes. Responsible for internal provider referral work queue monitoring, patient/prospect follow-up through the opt-in process. The Liaison will work with clinics and AM+ teams on site and act as the access point for patients, creating a cadence of touchpoints over the lifetime of the program. The position requires monitoring the pulse of the patient experience and skills in coordination, advocacy, problem resolution, and service recovery. It requires in-depth knowledge of health plans and benefits, and AltaMed health services offerings and navigation. Experience with outcomes reports/summaries and strong presentation skills. The Patient Relations and Retention Liaison requires exceptional time management and accountability with time in the clinic and some time in the corporate office (meetings, trainings, etc) Minimum Requirements
- High school diploma required
- AA/BA in healthcare (or related field), medical assistant, or additional relevant education is a plus.
- Minimum of 3 years working in a health care environment, health plans with the Medicare and/or senior market; knowledge of plan benefits required.
- Minimum of 3 years of experience working in health care coordination services, patient experience, and retention efforts with ethnically diverse populations is highly preferred.
- Bilingual English/Spanish proficiency required.
- Experience with CRM and EPIC is a plus.
