About the Role The Carolina Outreach team is looking for a compassionate, dedicated, and skilled Psychiatric Nurse (RN) to join our Assertive Community Treatment (ACT) Team. As an ACT Nurse, you’ll be at the heart of service delivery—partnering closely with clients, families, and an interdisciplinary team to ensure that every individual receives the care and support they need. You’ll oversee the medical and clinical components of treatment, blending your clinical expertise with a deep commitment to person‑centered care. Responsibilities Monitor and oversee all medical/clinical aspects of services delivered. Provide medication monitoring and documentation. Deliver psychopharmacologic and medical services within your scope of practice. Collaborate with ACT team members to monitor clinical status and response to treatment. Conduct crisis assessments, symptom management, and intervention planning. Provide individual counseling, substance abuse treatment, and psycho‑educational support. Assist in creating transition plans when clients move in and out of services. Qualifications Registered Nurse licensed to practice in NC by the Board of Nursing. Valid driver's license and current automobile insurance. Completion of DMH‑approved ACTT training. Demonstrated competence in providing ACT services prior to service provision.
