Field LVN Community Healthcare of Texas has provided hospice and palliative care services since 1996. Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them. Community Healthcare of Texas is currently recruiting for a Field LVN. $2,500 Sign-on bonus Position Summary Assist the RN Case Manager in the plan of care for the patient. Essential Functions Assists RN in case management of total plan of care of patient/support persons by coordinating with team members and communicating with IDG in accordance with organizational policies, procedures, and regulations that define the scope of practice. Complete, maintain, and submit accurate and relevant clinical notes regarding the patient's condition and care given. Record pain/symptom management changes/outcomes as appropriate. Administer medications and treatments as prescribed by the physician. Document nursing care and significant interactions with patients, support persons, and team members. Assess patient/family response to care and participate in follow-up with bereaved families with various types of contact. Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis. Participate in on-call rotation Position Qualifications Licensed Vocational Nurse licensed by the State of Texas, required Hold a valid driver's license and have a dependable means of transportation 1 year of nursing, hospice, or oncology experience preferred 1 year LVN experience in an acute care setting required.
