**CHOMP is a NICOTINE-FREE facility - all drug reports must have a negative result for nicotine/cotinine. If tested positive, will submit for review if traveler has been prescribed a nicotine patch. For all reports with no legitimate reason for positive nicotine/cotinine, the contract will be cancelled.

General Job Duties and Position Summary:
We are seeking a dynamic and skilled Physical Therapist to join our healthcare team, with a unique opportunity to split time between the Acute Inpatient Unit and a Skilled Nursing Facility (SNF). This position provides a challenging and rewarding opportunity to work with diverse patient populations, from those recovering from neurological injuries and surgeries in an acute setting to elderly patients with a variety of rehabilitation needs in the SNF.
The successful candidate will deliver high-quality, patient-centered physical therapy services, collaborate with multidisciplinary teams, and help improve patients' functional outcomes in both settings.
Education: Doctor of Physical Therapy (DPT) degree from an accredited program.


Licensure: Active state licensure as a Physical Therapist (or eligibility for licensure).


Experience:
Experience in neurological rehabilitation or acute inpatient care preferred.
Experience in geriatric rehabilitation and SNF care is preferred.
Skills:
Strong knowledge of neurological disorders and evidence-based rehabilitation techniques.
Ability to work independently and as part of an interdisciplinary team.
Excellent communication, interpersonal, and problem-solving skills.
Strong documentation and organizational skills.
Certifications: Current California PT license and CPR/BLS certification.


EMR: Epic


Shifts/Hours/On call requirements: 8 hours days 5 days a week, 2 weekends a month


Floating: Split time between Inpatient Rehabilitation Unit and Skilled Nursing Facility depending on needs of the unit


Dress Code: Hospital Provided Scrubs
