RN Purchased Referred Care Case Manager

The RN Purchased Referred Care (PRC) Case Manager will be responsible for providing case management services. This role involves managing patient care for individuals referred for services. The case manager will coordinate with various stakeholders to ensure quality care. Effective communication skills are essential for this position. A strong understanding of healthcare systems and case management practices is required. This position supports patient navigation through the healthcare continuum. The RN will collaborate with healthcare providers to optimize care outcomes. Attention to detail and strong organizational skills are crucial for success in this role. The role requires the ability to work independently and as part of a team. The candidate should have a patient-centered approach to care.
