Physician Assistant for In-home Visits

Signify Health, part of CVS Health®, is seeking board‑certified Physician Assistant/Physician Associate (PA) contractors to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. The role focuses on comprehensive home assessments and enabling members to better manage their health. Roles and Responsibilities Conduct in‑home visits that focus on a person’s overall health and well‑being, identifying potential barriers to care. Perform medication and medical history reviews, and a physical evaluation in a health plan member’s home. Use pre‑populated, patient‑specific forms for each evaluation. Do not prescribe medicine, order laboratory tests, or alter the member’s current treatment regimen. Accept scheduled in‑home visits with Medicare Advantage, Medicaid, or certain commercial health plans. Compensation and Benefits Earn up to $1,800 to $3,000 per week (average 6–9 visits daily, $70 per completed assessment). Increase per‑visit rate up to 160% with the Market Multiplier. Receive additional compensation via a pay‑per‑test model for visits where the member qualifies for diagnostic and preventive tests. Avail availability incentives based on weekly availability. Airfare, hotel, and rental car provided for eligible visits. Malpractice coverage. Work Schedule Shifts available Monday – Sunday with no nights or weekends required. Both short‑term and long‑term engagements are possible. Qualifications Board‑certified PA‑C (or equivalent Physician Associate) with a required state license. Medicaid license preferred, or willingness to enroll with support from Signify Health. Experience with in‑home health evaluations is beneficial. Independent contractor status (1099).
