Family Practice/Primary Care Nurse Practitioner

This Family Nurse Practitioner role focuses on delivering comprehensive, family-centered primary care across the lifespan, combining preventive services, chronic disease management, acute care, and patient education. Working in a community health setting, the provider supports individuals and families through direct clinical care, wellness promotion, counseling, and coordinated referrals while emphasizing culturally responsive, accessible healthcare. Dates/Schedule:

• Full-time Family Nurse Practitioner position (schedule not specifically listed)
• Community clinic-based role providing ongoing patient care
• Includes direct patient visits, follow-up care, health promotion events, and support group activities

Job Details & Responsibilities:

• Evaluate, diagnose, and treat chronic conditions, acute illnesses, and minor injuries
• Perform physical exams, primary care screenings, Pap tests, laboratory testing, and office procedures within scope
• Prescribe and dispense medications according to protocols and available resources
• Provide patient counseling on health, lifestyle, development, and disease prevention
• Conduct support groups related to physical and emotional wellness
• Coordinate preventive health initiatives such as screenings, vaccinations, and educational outreach
• Manage overall patient care and coordinate follow-up services
• Refer patients to physicians, specialists, and additional medical services as needed
• Orient new staff regarding departmental services and community healthcare resources
• Maintain accurate Electronic Medical Record documentation

Required Skills & Qualifications:

• Licensed Nurse Practitioner in California with valid California RN license
• Current DEA and BLS certification
• Master s degree in related field preferred
• Minimum 1 2 years of community clinic experience required
• At least 6 years of RN experience related to family health required
• Recent Electronic Medical Record (EMR) experience
• Community Health Center experience strongly preferred
• Specialty clinical experience preferred
• Bilingual Spanish/English required
• Strong commitment to the American Nurses Association Code of Ethics, patient advocacy, and family-centered care.
