Geriatric Primary Care Physician — Day Shift

Trinity Health Life at Kinder Park is seeking a Primary Care physician to provide comprehensive medical care to PACE participants. You will develop care plans, perform assessments, and coordinate with an interdisciplinary team to optimize health outcomes. The role requires board certification in Internal Medicine or Family Practice, active licensure, and experience with frail or elderly populations.
