Physician

OnPoint Medical Group is seeking a Physician to provide comprehensive, patient-centered care within our coordinated primary and specialty care network in the Denver metro area. In this role, you will evaluate and treat patients across the lifespan, manage chronic and acute conditions, and deliver preventive and wellness services. You will collaborate closely with a multidisciplinary team, contribute to quality improvement initiatives, and build long-term relationships with patients and families while supporting our mission of compassionate, community-focused care. Responsibilities Provide comprehensive primary and/or specialty medical care to a diverse patient panel Conduct patient evaluations, develop differential diagnoses, and manage acute and chronic conditions Deliver preventive care, wellness visits, and age-appropriate screenings Document encounters accurately and efficiently in the EHRCoordinate care with specialists, behavioral health, and ancillary services Collaborate with nurses, MAs, and other team members in a patient-first model Educate patients and families on treatment plans, medications, and lifestyle changes Participate in quality improvement, population health, and care coordination initiatives Maintain compliance with clinical guidelines, practice policies, and regulatory standards Contribute to a respectful, collaborative, and community-focused clinic culture Required Skills Clinical diagnosis and treatment Electronic Health Records (EHR) documentation Chronic disease management Preventive and wellness care Care coordination and referral management Patient counseling and education Evidence-based medicineMinor in-office procedures Interdisciplinary team collaboration Quality and performance improvement #J-18808-Ljbffr OnPoint Medical Group
