Physical Therapist Physical Therapy

Schedule: 8 hours shift

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40 per week

Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license. Must have reliable transportation and insurance
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience - Preferred
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program

Roles & Responsibilities:

• Perform clinical assessments to identify patient needs and appropriateness.
• Document, prepare/review and complete a Plan of Care (POC), progress notes, and clinical record documentation on an ongoing basis.
• Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessments.
• Assist/instruct patient/family in transfer techniques and the use of equipment in a safe manner.
• Provide appropriate patient/family education regarding therapeutic procedures based on the patient's age, condition, culture, and learning readiness.
• Collaborate with an interdisciplinary team and consult with physicians as needed.
