Physical Therapist Physical Therapy
Schedule: 8 hours shift
Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40 per week
Requirements:
- Current State License as a Physical Therapist
- Proof of current CPR certification
- Current driver's license. Must have reliable transportation and insurance
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program
Roles & Responsibilities:
- Perform clinical assessments to identify patient needs and appropriateness.
- Document, prepare/review and complete a Plan of Care (POC), progress notes, and clinical record documentation on an ongoing basis.
- Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessments.
- Assist/instruct patient/family in transfer techniques and the use of equipment in a safe manner.
- Provide appropriate patient/family education regarding therapeutic procedures based on the patient's age, condition, culture, and learning readiness.
- Collaborate with an interdisciplinary team and consult with physicians as needed.
