Minimum Qualifications

• Master’s degree in Physical Therapy required; Bachelor’s or Doctoral degrees acceptable if accredited and APTA-approved.
• Active New Hampshire Physical Therapy license required prior to start.
• BLS certification from AHA or ARC may be required.
• Home health experience preferred; must be able to work in patient homes and community settings.
• Experience with HCHB EMR preferred.
• Ability to meet productivity expectations and weekly hour guarantee of 32–40 hours.

Preferred Qualifications

• Prior home care experience and familiarity with home health documentation workflows.
• Comfort with mileage reimbursement processes for travel between patient visits.

Assignment Details

• Shift hours: 08:00–16:00 (client notes also reference 08:00–16:30 for schedule alignment).
• Weekly hours: 32–40 guaranteed.
• Coverage area: Epping care territory; clinician may travel within assigned coverage area.
• On-call/weekend requirement: no.
• Mileage reimbursement provided for travel between first and last patient visits.
