Physical Therapist - Full-Time

Join Our Team as a Full-Time Physical Therapist (PT) We are seeking a dedicated and compassionate Physical Therapist (PT) to join our in-house team of outstanding professionals. In this role, you will provide person-centered care to help residents achieve positive outcomes and improve their mobility, strength, and overall quality of life.

Your Impact as a Physical Therapist (PT)
- Consult with patients to assess their physical condition and functional limitations
- Evaluate and interpret patient assessments and test results
- Develop individualized treatment plans using a variety of therapy techniques
- Create personalized rehabilitation programs tailored to each patient's goals
- Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing
- Ensure all treatments are delivered in accordance with the established plan of care
- Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team
- Provide clinical support and education to patients, family members, and caregivers
- Collaborate with interdisciplinary team members to ensure high-quality patient care
- Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
- Comply with all applicable rules, regulations, and procedures
- Attend required meetings as designated by the Director of Rehab
- Active and valid Texas Physical Therapist license
- In good standing with all regulatory agencies and licensing boards
- Skilled Nursing Facility (SNF) or long-term care experience preferred
- Current knowledge of physical therapy treatment practices
- Experience with Net Health Electronic Documentation System preferred
- Ability to manage patients with varying personalities and needs
- Strong communication, collaboration, and clinical decision-making skills
- In-house Rehab (non-contracted)
