Inpatient DietitianPlans, develops, and provides diet interventions, medical nutrition therapy, nutrition care, and preventive counseling to individuals and groups. Services are provided in all areas of primary and specialty patient/resident care including acute care, skilled nursing, out-patient, and community settings. Provides medical nutrition therapy services and monitors the nutritional care of clients/patients/residents of all ages including neonatal, pediatric, adolescent, adult, and geriatric as assigned. Determines nutritional status through nutritional assessment, individualized care plans, provider order recommendations, and implementation of nutrition therapy. Nutrition therapy includes verbal orders, diet modification, nutrition support, and oral nutrition supplements. Education and counseling for disease and nutrition risk management are provided along with evaluation of outcomes of medical nutrition therapy interventions. Optimizes self-management education and skills for clients/patients/residents with chronic conditions. Recognizes, evaluates, and provides for nutritional needs based on cultural, ethnic, and religious beliefs. Evaluates treatments or procedures for effectiveness using an evidence-based approach. Works on nutrition care protocols for the assigned area and acts as a leader for the healthcare team in the implementation of nutrition therapies.
