Position Overview

• Primary care and wellness clinic coverage with proactive care, early intervention, and chronic disease management (per order details)
• No nights, no weekends, no walk ins (per order details)
• Patient ages: 2 plus (per order details)

Required Qualifications

• Active, unrestricted license in Arizona as an FNP or PA (per order details)
• National certification as an FNP or board certification as a PA (per order details)
• DEA registration (per order details)
• Ability to work autonomously in the state without a supervising physician requirement (per order details)
• Strong HIPAA and confidentiality standards (per order details)
• Primary care experience: 2 plus years preferred (per order details)

Clinic Responsibilities

• Prescribe medications as clinically appropriate (per order details)
• Patient education and long term wellness coaching (per order details)
• Collaborate with clinical teammates and contribute to a positive clinic culture (per order details)
