divh2Wellness Director/h2pOur Assisted Living Community Amber Court of Pelham Gardens, located in a beautiful residential area in the Bronx, is seeking a Registered Nurse for the Position of Wellness Director to join an amazing team./ppResponsibilities:/pulliPre-Assessment and assessment of new admissions re-admissions to the adult care facility/liliRequires oversight of case management activities; case manager of record./liliWork with the health care team to assure that our residents receive the best quality care/liliImplement policies and procedures related to the wellness of the residents, including infection control prevention/liliPerform and timely submit UAS Assessments via the Health Commerce System/liliOrganize resident care information and participate in Quality Improvement and other quality initiatives/liliProvide ongoing guidance to the care team on Wellness processes, regulations and initiatives/liliPerform duties in accordance with Assisted Living NYS DOH guidelines/liliPerform duties in accordance with licensure as a Registered Nurse in New York./li/ulpRequirements:/pulliCandidate must be a Licensed Registered Nurse (RN) currently registered in New York State./liliCandidate must have the actual experience to Submit UAS Uniform Assessments via the Health Commerce System/liliCandidate should have a warm and compassionate nature, able to interact with the geriatric population and staff in an empathetic manner/liliAble to work as part of a team and contribute to the success of the company/liliPrior experience in Assisted Living, Skilled Nursing or a related senior focused environment/liliPrior management experience preferred/liliCandidate must have completed UAS training/liliExcellent computer skills needed./li/ulpThis position reports to the Regional Director of Nursing and the facility Executive Director/ppJob Type: Full-time/ppBenefits:/pulliPaid time off/li/ulpExperience:/pulliAssisted Living Management: 1 year (Preferred)/liliUAS : 1 year (Required)/li/ulpLicense/Certification:/pulliRN License in New York State (Required)/li/ul/div
