About Our Facilities

Our organization operates Skilled Nursing Facilities (SNFs) that offer:

• Long-term care: Support for residents needing ongoing skilled assistance and compassionate daily help.
• Short-term, sub-acute rehabilitation: Services for patients recovering from surgery, illness, or injury.
• Collaborative care: An interdisciplinary team approach involving nursing, therapy, and medical staff working in unison.

The patient demographic may vary slightly by facility. As a therapy professional, you will play a vital role in helping patients achieve functional improvements and enhance their quality of life in a supportive, patient-focused environment.

Position Overview: Physical Therapist (PT)

The Physical Therapist provides direct rehabilitation care in physical therapy to patients based on physician referrals, adhering to all relevant laws, regulations, and organizational standards.

Qualifications

• Graduation from an accredited physical therapy program (BSPT, MSPT, or DPT; those graduating in 2003 or later must hold an MSPT or DPT)
• New graduates are welcome to apply
• Participation in community health initiatives/projects as appropriate
• Proficiency in Microsoft Word, Excel, and email communication

Key Responsibilities

• Engage with patients, families, and support departments effectively
• Understand and apply the practices, procedures, laws, regulations, and guidelines relevant to post-acute care facilities
• Implement and interpret departmental programs, goals, objectives, policies, and procedures
• Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation
• Maintain confidentiality of all proprietary and confidential information
• Adhere to company policies, including those related to harassment and compliance
• Exhibit integrity and professionalism by following the organization’s Code of Conduct and completing required compliance training
• Follow established physical therapy treatment plans for patients
• Supervise and assess the care provided by PTAs, Rehab Aides, and PT and PTA students
• Utilize therapy software accurately and appropriately
• Provide excellent customer service and maintain a positive attitude towards patients
• Read, write, speak, and understand the English language proficiently
