• Conduct thorough evaluations of patients in either inpatient or outpatient environments and develop tailored treatment plans.
• Implement physical therapy interventions focused on improving mobility, strength, balance, and function.
• Collaborate proactively with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes.
• Educate patients and their families about therapeutic objectives, discharge planning, and home exercises.
• Maintain detailed documentation of evaluations, treatments, and progress as per regulatory and facility guidelines.
• Engage in care team rounds and interdisciplinary conferences to promote coordinated care.

In terms of education, we're looking for candidates with a Doctorate in Physical Therapy (DPT) or a closely related accredited degree. State licensure as a Physical Therapist is required, and while a CPR certification is preferred, any specialized training in areas such as neurology or geriatrics is a bonus. Successful candidates will possess previous experience, exhibit a strong capacity to treat patients with intricate medical and mobility challenges, demonstrate excellent communication skills, and be comfortable with digital charting and clinical documentation systems.
