LPN/ LVN - Home Health Nurse

Pay: $30.00 - $35.00 per hour Job description Job Summary We are seeking a compassionate and dedicated Registered Nurse for our Home Health department. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong background in various nursing specialties, excellent patient assessment skills, and the ability to work independently while collaborating with a multidisciplinary team. Duties Conduct comprehensive patient assessments and develop individualized care plans. Administer medications and treatments as prescribed, including IV infusion and injections. Monitor patient vital signs and report any significant changes to the healthcare team. Provide education to patients and their families regarding health management and care plans. Assist with activities of daily living, including tube feeding and catheter care. Maintain accurate medical documentation in compliance with HIPAA regulations. Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care. Implement infection control measures and ensure a safe environment for patients. Support patients with developmental disabilities, Alzheimer’s care, dementia care, and other specialized needs. Participate in discharge planning and case management to facilitate smooth transitions of care. Requirements Current Registered Nurse (RN) license in the state of practice. Experience in home health or community-based nursing preferred. Proficiency in EMR/EHR systems such as Epic, Cerner, or Athenahealth. Strong clinical skills in areas such as critical care, geriatrics, pediatrics, acute care, or hospice care. Familiarity with ICD-9/ICD-10 coding and medical terminology is advantageous. Ability to perform phlebotomy, venipuncture, catheterization, and other nursing procedures. Knowledge of pain management techniques and acute pain management practices. Experience working in various settings including hospitals (Level I/II trauma centers), nursing homes, assisted living facilities, or outpatient clinics is preferred. Excellent communication skills and the ability to work effectively with diverse populations. Commitment to ongoing professional development and adherence to NCQA standards.
